Physicals for student athletes in the 2008-2009 school year must occur after June 1, 2008, per PIAA. Re-certification of a physical from before that date is no longer possible. For more information please read the attached.

IMPORTANT NOTE: Physicals for sports in the 2007-2008 school year must be dated AFTER June 1, 2007. If your child is going to be participating in a sport, a physical examination is required BEFORE they may practice. Have your child's doctor complete the Comprehensive Initial Pre-Participation Physical Evaluation (CIPPE) form at the exam. If your child's latest physical examination occured before June 1, 2007, you and your child's doctor must complete the Re-Certification Form. See below.

If your child's latest physical examination occured before June 1, 2007, you and your child's doctor must complete the Re-Certification Form. This form must also be dated after June 1, 2007 to participate in sports for the 2007-2008 school year. (The Re-Certification Form is page 5 & 6 of the CIPPE form.)

The Re-Certification form should be used for students who previously completed the CIPPE form but then suffered an injury that caused them to miss more than 25% of the season, or students who missed school for more than ten days due to illness, injury or surgery.
