Online Service: CBSD has subscribed to an online service to post athletics schedules to the internet. Go to www.highschoolsports.net and search for Holicong to locate team schedules, driving directions or sign up for email notification of any changes in schedule.
Physical Form

Dear Student and Parents,
PIAA regulations governing interscholastic athletics require student athletes to undergo proper medical examination prior to their participation. The determination whether your son or daughter is medically eligible for participation can be made most accurately by the physician who knows the student best – your family doctor or pediatrician.
To tryout/practice for sports you will need the PIAA’s Comprehensive Initial Pre-Participation Physical Evaluation which is required for all students who wish to participate in a school sport. The form is not valid for school sports unless it is dated June 1, 2008 or thereafter and will be valid for the entire school year. The Green Code Of Conduct Form must be signed by the parent/guardian and the athlete and will also be valid for the entire school year. You must also complete the Yellow Emergency Contact Form and the Blue Athletic Department Eligibility Form before the start of each season. All four properly completed forms must be on file prior to participating in any practice/try-out session.
If a student participates in another sport during the same school year, the student will only need to have section 6 recertification form of their sports physical completed by their personal physician if they’ve suffered a serious injury/illness that caused the student to: miss ten (10) days of school; miss 50% of games/contests; or suffer a season-ending injury. Otherwise, the original physical is valid for a calendar year and you will only need to complete the athletic department eligibility form, section 5 of the sports physical and the emergency contact form before participating in another sport.
