Any student interested in trying out for a sport at Lenape must have a completed physical or re-certification turned into their respective coaches on the first day of tryouts.
2007-2008 School Year Sports Physical Information:
Dear Parents:
PIAA regulations governing interscholastic athletics require student athletes to undergo proper medical examination prior to their participation. The Central Bucks School District requests that all student athletes be seen by their family physician. The school physician will continue to monitor our athletic teams, but will no longer be the primary physician conducting sports physicals. The school physician will provide physical examinations for students in need of financial assistance.
Students who wish to participate in PIAA sports must obtain a Comprehensive Initial Pre-Participation Examination (CIPPE) Form from the athletic office or on-line at www.cbsd.org or www.piaa.org. The parent/guardian and the student’s personal care physician should complete this CIPPE form. The form must be returned to the school prior to the start of the athletic season. The CIPPE form is valid for one (1) academic year, June 1st – May 31st.
If a student participates in multiple sports during the same school year, the student-athlete’s parents will need to complete Section 5 of the CIPPE form (re-certification) prior to the start of the new season. A physician’s signature is not required for re-certification unless the student-athlete has suffered a serious injury/illness that caused the student-athlete to: 1). Miss ten or more days of school; 2). Miss 50% or more of games/contests; or 3). Suffered a season-ending injury.
