SCOLIOSIS SCREENING FOR 7TH GRADE STUDENTS
Please note that scoliosis screening will be occurring on Monday and Tuesday, February 8th and 9th. All seventh grade students will be screened, privately, in the boys and girls locker rooms, in the gym. If you do not wish your child to be screened, please send a note in writing prior to February 3rd and have your child bring it to the Nurse's Office.

School Dental Examinations will be held on Friday, March 26, 2010. All 7th grade students must have a current dental report on file. Students who have not returned the form will be receiving a permission to be seen by the school dentist in their report card envelope on February 2nd. he
**September 30, 2009 Lunch Program Information**
All students participating in the Free/Reduced lunch program must have a CURRENT, 2009, application on file. If your child was on the program last year and a new application was not completed, the grace period has expired. As of October 1st, only those students with new applications on file will be service through this program.
Reminder: October 15th DUE DATE
All physical and dental forms must be completed and returned to the nurse's office no later than October 15th. Student's who have not returned the forms will not be allowed to participate in extra-curricular activities such as sports, dances, etcetera.

All New Students: Dental and Physical Examination Reports
A blank form can be retrieved from the parent resources selection on the home page website, www.cbsd.org, or ask your child to stop into the nurse's office and pick one up.
